List and Description of Forms

Registration Form
This is the registration form that must be filled out before a child may participate in The Village's activities.

Individual Entry Form
This is the form that must be filled out for a child to participate in an individual track meet.

Team Entry Form
This form is for coaches to fill out to have all students on their team registered as part of that team.



Don't forget to pay your AAU membership fee to participate in any of their track & field events!
